Around 50,000 people in North Yorkshire and York live with a long term health condition such as Chronic Obstructive Pulmonary Disease (COPD), Chronic Heart Failure and Diabetes.

This website is about how telehealth technology is helping people in North Yorkshire and York to take control of their condition and reduce the risk of unplanned admissions to hospital.

The website features videos of patient stories and information for health professionals, as well as Q&As and plenty of facts and figures.
